The mother's maiden name is needed to follow the policy and procedures for recording the sacrament in the baptismal register as outlined by the Catholic Church's Code of Canon Law. That is, the names of the parents recorded in the register will be those given to them at birth. This means the mother's married name is not recorded in the baptismal register and will not appear on the baptism certificate you receive for your child(ren).

Responsibilities of the Parents
As part of the baptism ritual, the parents requesting baptism for their child will be asked if they are ready and willing to accept responsibility of raising their child in the practice of the Catholic faith: raising them to keep the commandments as Christ taught – loving God and their neighbor. To fulfill this obligation, it is important that the parents nurture their child's faith at home and within the Catholic worshipping community. The Godparents and the local Catholic community have the opportunity and responsibility to assist the parents in doing this.

Information about Baptismal Godparents
Because of the importance of their role, baptismal Godparents must be:

1. Fully initiated Catholics. They themselves must have celebrated the sacraments of:
a. Baptism
b. Confirmation
c. First Eucharist

2. Be at least 16 years of age

3. Be practicing Catholics who lead a life in harmony with the faith and the role to be undertaken.

4. If married, they should have been married in a Catholic ceremony. (Code of Canon Law, c. 874)

5. Only One Godparent Is Required. If you wish two individuals to serve as Godparents, one must be male and the other female.  

6. If a Godparent is not able to be present, that Godparent may request a proxy to stand in his/her place.

7. A baptized non-Catholic is not eligible to serve as a baptismal Godparent. However, he or she may serve as a Christian Witness. The Christian witness participates in the celebration and stands alongside the Godparent during the baptismal ceremony.

It is the responsibility of the parents to submit to the Pastor in writing – prior to the date of baptism – that the chosen Godparent(s) fulfill(s) all of the above requirements.
